To request an appointment, please fill out the form below and hit the 'send' button at the bottom. Click the calendar icon for a pop-up calendar to help you select a date; remember to fill in a second date in case you first choice isn't available. Please try to give us 48 hours to receive and respond to the appointment. Remember to supply a home and/or work number, or an e-mail address where we may contact you.

The initial 30 minute consultation is free. Should you decide to retain us, we gladly accept payment in cash, checks and all major credit and debit cards.
